Proteinuria is a condition in which protein is found in the urine, and is often defined as a quantity in excess of 300 mg per day. It is linked with cardiovascular and renal disease and is an indicator of end organ damage in patients with hypertension. Protein should not normally pass out in the urine in detectable quantities, but when the filtration system of the glomeruli is damaged, small proteins such as albumin can pass through.

The causes of proteinuria can vary, but some of the most common include high blood pressure, infection, reflux nephropathy, diabetes, glomerulonephritis, and minimal change nephritis. Symptoms of proteinuria are usually absent, but in more serious cases, such as nephrotic syndrome, there may be edema, ascites, hydroceles, and pleural effusions.

Diagnosis of proteinuria is done through a urine sample, which is analysed for levels of protein and creatinine. An albumin-creatinine ratio (ACR) of 3-30 does not normally require action, but an ACR of higher than 30 indicates considerable leakage of protein through the kidneys. The size and shape of the kidneys may be measured in the X-ray department with an ultrasound, and a kidney biopsy may be required to make a firm diagnosis of the cause of proteinuria.

Treatment of proteinuria is dependent on identifying and treating its underlying cause. In mild or temporary proteinuria, no treatment may be required, but medicines are given for high blood pressure and diabetes to avoid the progressive kidney damage causing the proteinuria. Water retention can be treated by reducing the amount of salt. If proteinuria is high, it is likely to need treatment by a specialist from the outset and further investigation may be required. 🤔
